Denver Department of Public Health & Environment and Denver Public Health are collaborating with local businesses and organizations to investigate and control COVID-19 outbreaks. Please report an outbreak of COVID-19 in a Denver workplace or organization and we will review the information submitted. After review, you or the workplace will be contacted by a representative to will discuss next steps. If you are a workplace or facility experiencing a confirmed or suspected COVID-19 outbreak, please use the COVID-19 line list template to begin recording information about your staff, guests, or residents of your workplace/facility that have developed COVID-19-like symptoms or have tested positive for COVID-19. Additional guidance for COVID-19 outbreaks in the workplace can be found on the Colorado Department of Public Health and Environment’s Workplace Outbreak Guidance site.
Long-term care facilities (Nursing Homes, Assisted Living Facilities, etc.) should send an email directly to DDPHEOutbreak@denvergov.org to report any cases or outbreaks in your facility instead of using the survey above.
